The Care Choices Palliative Care Program improves the quality of life for patients undergoing curative treatment for a life-threatening illness and their loved ones through a combination of pain and symptom management and other medical, emotional, practical, and spiritual support services.
Care Choices is appropriate for anyone with a complex or serious illness, whatever the diagnosis and stage of their treatment.
HEALTH CARE TEAM
Two physicians and nurse practitioners oversee the program. The patient is an important member of the health care team and plays an active role in the development of the care plan. Clinicians remain sensitive to patients' personal, cultural and religious values, beliefs, and practices.
